New forms to be used for students to be picked-up and driven home from away contest.

Needs to be completed, signed and turned in for every athletic event where parents are driving students’ home.
Attachment C
Needs to be filled out one time unless there is a change of driver and/or car. It also needs to include copies of the parent driver’s license and auto insurance.
