Athletic Paperwork
Athletic Clearance will be up and running.
Please use the AC User Handout as a guide.
Make sure to select the following:
- Year: 2023-2024
- School: North Hollywood (CIF-LA)
- Sport/s: Select ALL SPORTS you plan on trying out for during the school year.
The 2023-2024 Athletic Packets will be submitted electronically. All athletes will be required to still turn in the following items to their Coach.
- Physical Form
- Proof of Insurance
- Emergency Card
- Conformation page from https://www.homecampus.com/login with signature verifying completion of all documents.
